3. Simply mail your completed card, including proof of purchase (ie. receipts - either the originals or a legible photocopy) for the 5 titles you have bought and details of your choice of FREE 6th CD, to:

We will send your FREE 6th CD, subject to availability, to your nominated store for collection.

Should the CD you request be unavailable, we will contact you to arrange for an alternative title.
The value of your FREE 6th CD cannot exceed the retail price of the least expensive title you have purchased as shown on your receipts.

If a Connoisseur Collection title is not in store, please allow 3-4 weeks for it to be ordered and sent to your participating store. Orders for titles that you need before Christmas 2008 need to be placed with Select Audio-Visual Distribution by close of business on Friday 29 November 2008. We fulfill orders as quickly as possible, but may not be able to supply titles ordered after this date until the New Year.
